Summary


The HIV Medical Case Manager is responsible for providing case management services to persons living with HIV. The case manager shall work closely with persons diagnosed with HIV to provide medical case management, adherence counseling and support services.


This position is accountable to the HIV Care Program Manager and functions as part of the broader Case Management team. The HIV Case Manager must hold a bachelor’s degree in Social Work, Psychology, Sociology or Human Development. At least two years of relevant experience is preferred. Lived experience is valued and populations that have faced historic barriers to employment are encouraged to apply. The HIV Case Manager’s duties are as follows:


Specific Duties & Responsibilities


 HIV Case Management:


  • Assists with the design, planning, development and implementation of programs to support people who use drugs.
  • Provides Case Management services including intake and assessment of HIV positive clients and their needs.
  • Coordinates benefits and care and makes referrals in satisfaction of client care plans.
  • Meets or exceeds weekly client encounter targets based on caseload.
  • Advocates on behalf of clients with physicians, substance use counselors, housing authorities, and other appropriate agencies.
  • Maintains and regularly updates client records; manages Medicaid and Ryan White eligibility and appropriate records.
  • Ensures the confidentiality of all client records and transactions.
  • Works in conjunction with local community resources and services to aid client in meeting basic needs.
  • Reports incidents of neglect and abuse and contributes to the client grievance process.
  • Job may require travel to meet with clients and on some occasions transporting clients.
  • Actively participates in the agency’s Case Management Team.
  • Serves as an advocate for Health Equity Alliance and HIV-related issues.
  • Assists in agency fundraising events.
  • Other duties as assigned by the Program Manager.


Benefits and Salary


Salary Band: $38,500 - $42,000
